I hope this communication finds you well and that your child has enjoyed being back at school this term.
As you may be aware, the Government of Western Australia now requires schools to hold records of their enrolled students Immunisation History.
As such, we require an updated Immunisation History Statement for all students, one that includes your child’s latest 4 year old immunisation details.
Please see the attached flyer on how to download a copy of the Immunisation History Statement via the MyGov website.
If you haven’t already could you please forward a copy of this updated statement to admin@stdominics.wa.edu.au by the end of Term 2.
