I hope this communication finds you well and that your child has been enjoying their time in Kindergarten this year.
As you may be aware, the Government of Western Australia now requires schools to hold records of their enrolled students Immunisation History.
As such, we now require an updated Immunisation History Statement for all our Kindy students.
Please see the attached flyer on how to download a copy of the Immunisation History Statement via the MyGov website.
If you haven’t already could you please forward a copy of the updated statement to admin@stdominics.wa.edu.au by Friday, 7th August.
